How do Revive-ify Wellness consultations work?
Booking your first appointment is easy. Just click the "Get Started" button to request an appointment. Before your consultation, you’ll complete a brief set of intake forms. You’ll then receive a secure link to join your virtual visit with Nurse Practitioner Kendall Schoenekase. If a prescription is recommended, your medication will be shipped directly to your doorstep within a few days.
Is my information confidential?
Absolutely. Your personal health information is 100% confidential and protected under HIPAA. Only our licensed medical team and prescribers can access your records. For more details, refer to our Privacy Policy.
How often will I receive a shipment?
Once prescribed, your medication is shipped directly to you within 3–10 business days. Be sure to schedule your follow-up appointments at least one week before running out, especially if you’re on a monthly check-in plan.
How much do medical weight loss services cost?
Initial consultations cost $150 (25% off this for referring a friend). Monthly follow up visits are $75 (50% off this for referring a friend). Prescription prices vary by the type of medication and pharmacy cost. You can expect to pay around $400 for a 2.5 MONTH SUPPLY of generic Ozempic, including all supplies and shipping! Larger vials are also available for purchase, which are even more cost effective.
How much do longevity / NAD+ services cost?
Consultation (Required Annually): $75. NAD+ prescription costs vary by your location and the amount of medicine ordered.
What forms of payment do you accept?
We accept credit card (including a 3.5% processing fee), Zelle, Venmo and even Cash App.
Who is eligible for medical weight loss treatment?
Most adults are eligible! If you're struggling with weight, cravings, energy levels, or metabolic health, medical weight loss can be a powerful tool to support your goals. Book a consultation to see if it’s right for you.
Who is eligible for longevity treatment?
There are currently no known contraindications or drug interactions, making it safe for most adults. However, NAD+ therapy is not recommended for:
Children or pets
Individuals who are pregnant or breastfeeding
Anyone with known allergies to NAD+ or its ingredients
Do you accept insurance?
No, we do not accept insurance at this time. However, our pricing is designed to be affordable and often comparable to or better than insurance-based models — with no surprise fees.
Can I use my HSA/FSA card?
Yes! Many patients use their HSA or FSA cards to pay for services and prescriptions. We recommend checking with your provider to ensure coverage.
What if I have questions between visits?
As part of your coaching plan you’ll have access to secure messaging for any questions related to your medications, side effects, or progress.
